Regarding Your Medical Records
Your medical records will remain with Bellaire Dermatology Associates unless you instruct us otherwise. If you wish to have your records transferred to Dr. Gerber or to any other provider, you may request a copy by completing and returning a Medical Records Request Form. This form is also available by contacting our office at 713-661-4383 or by visiting our office at 6565 West Loop South, Suite 800, Bellaire, Texas 77401. You or your new provider may also request your records by phone or in writing.
There is no charge for transferring your records to a new treating physician. You are entitled to one copy of your medical records at no charge for the purpose of continuity of care.
